A presidential panel, known as the Rogers Commission, was formed to explore the causes of the tragedy. The commission included astronauts Neil Armstrong (far left, first row) and Sally Ride (second from right, first row). The panel traced the disaster to a failure in the O-rings sealing a joint on one of the rocket boosters.
