It's the renewal card our circulation department sends you to advise you that your subscription to TIME is up for renewal. All of the card's perforations and code numbers refer to you. An explanation of them follows:

A. The code, giving your state, city, street address, and the first and last initials of your name (e,g., the F and W in the subscriber's name above).
